About the position

The Case Manager II position at UMass Memorial Health is a full-time role focused on providing comprehensive care coordination for clients within the outpatient mental health program. This position is integral to the Community Health Link (CHL) team, which emphasizes a collaborative approach to mental health care. The Case Manager will implement program policies and procedures to ensure daily structure and compliance with licensing standards. They will assess clients' physical, social, educational, legal, and service needs, coordinating care with various inter-agency and community-based providers. In collaboration with a multi-disciplinary treatment team, the Case Manager will develop and update integrated treatment plans that address all service needs. They will also create comprehensive aftercare plans in partnership with clients and service providers, ensuring a smooth transition and continuity of care. Outreach efforts will be a key component of this role, as the Case Manager will work closely with other providers to facilitate referrals and may conduct admissions assessments as needed. The position requires attendance at various meetings, conferences, and seminars, demonstrating a commitment to quality improvement in daily operations. The Case Manager will maintain a professional environment that respects diverse views and approaches, contributing to a culture of professionalism and acceptance. Regular attendance and the ability to perform related duties as required are essential to this role.

Responsibilities

  • Implements program policies and procedures to maintain daily structure and compliance with licensing standards.
  • Assesses clients' basic physical, social, educational, legal, and service needs and coordinates care with inter-agency and community-based providers.
  • Works with a multi-disciplinary treatment team to develop and update integrated treatment plans and recommendations.
  • Develops comprehensive aftercare plans while collaborating with clients and multi-service team providers.
  • Conducts outreach and creates pathways for referrals, working closely with other providers to receive referrals and may conduct admissions assessments.
  • Complies with established departmental policies, procedures, and objectives.
  • Attends meetings, conferences, and seminars as required or directed.
  • Demonstrates use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches, contributing to an environment of professionalism and acceptance.
  • Maintains regular, reliable, and predictable attendance.
  • Performs other similar and related duties as required.

Requirements

  • Bachelor's degree, preferably in psychology or human services.
  • Satisfactory CORI background check.
  • Ability to develop a working knowledge of CHL's service lines to triage individuals being assessed for services.
  • Excellent oral communication and computer skills.
  • Ability to multitask and possess time management skills with a keen sense of prioritization and follow-through.
  • Requires personal and professional accountability, self-management, and initiative.
  • Professional integrity and cultural competency.
  • Ability to use office equipment, including copy machines, computers, printers, and telephones.
  • Ability to organize complex tasks and meet deadlines.

Nice-to-haves

  • Prior experience in a behavioral health or social services setting.
  • Basic knowledge of CHL services and insurance requirements and reimbursement for these services.
  • Working knowledge of Electronic Health Records (EHR).
