About the position

The Case Manager II at Heartland Family Service is responsible for providing intensive case management services to clients within the Bridges programs, which include Transitional Living and Crisis Care. This role involves facilitating supportive services, responding to crisis situations, and ensuring clients receive the necessary resources to achieve stability in their lives. The position requires strong decision-making, problem-solving, and interpersonal skills, and is set in a collaborative, team-oriented environment focused on trauma-informed care.

Responsibilities

  • Responds to crisis care referral calls and assesses individuals by gathering information to provide appropriate application verdicts while maintaining confidentiality.
  • Completes enrollment/intake on eligible transitional and crisis clients.
  • Documents client activity, service plan progress, and needs per established record-keeping procedures.
  • Provides problem-solving and crisis intervention services to empower clients towards self-sufficiency.
  • Screens for mental health and substance use treatment needs and links clients to appropriate services.
  • Assists clients in locating, obtaining, and retaining suitable housing through housing search and placement services.
  • Develops supportive relationships with clients and responds to their needs during shifts.
  • Ensures required information is collected, entered in prescribed formats, and reported in a timely manner.
  • Provides program information and referral services to clients.
  • Monitors client self-administration of medication and follows procedures for recording.
  • Obtains and maintains certification to administer non-parenteral medications in Iowa.
  • Accurately records client outcomes on designated databases.
  • Psycho-educates clients on life, social, and coping skills, meeting with clients for weekly Life Skills Inventory coaching sessions.
  • Demonstrates knowledge of current community and HFS programs and resources to meet client needs, including connections to mainstream benefits and Integrated Health Home services.
  • Utilizes trauma-informed care, person-centered, and culturally competent principles in providing services to clients.
  • Evaluates efficiency and effectiveness of community service providers to ensure quality care for clients.
  • Orients clients to Bridges protocols and addresses non-compliance to create a safe environment.
  • Conducts client room safety checks and room searches per program protocol.
  • Responsible for maintenance and upkeep of the facility.
  • Works independently with little supervision and as part of a team.
  • Displays a courteous and caring attitude to clientele, volunteers, and visitors.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • Develops personal and program-related skills through participation in training opportunities.
  • Creates, maintains, and shares a dynamic self-care plan.
  • Performs other program-related duties as assigned.

Requirements

  • High School Diploma
  • Two years of experience in behavioral or mental health services
  • One year of experience working with adults with serious mental illness or substance use
  • Valid driver's license and acceptable driving record

Benefits

  • Competitive hourly wage between $17.95 and $21.40 based on experience
  • Part-time work schedule of three shifts per week (24 hours)
