About the position

The Case Manager II position at Frontier Health focuses on coordinating services for individuals with developmental disabilities (DD) within a designated caseload. This role involves the administrative maintenance of all case records and executing various case management duties assigned to the case manager. The primary objective is to ensure that individuals receive the necessary support and services tailored to their unique needs. The case manager will engage in a comprehensive process of service coordination, which includes assessing the service needs of individuals and families, planning and formulating Individual Service Plans (ISPs), linking individuals to appropriate resources, and monitoring the effectiveness of the services provided. The case manager is expected to conduct VIDES assessments to determine eligibility for waiver services and participate in Supports Intensity Scale (SIS) assessments. In addition to these responsibilities, the case manager will advocate for individuals by facilitating constructive relationships with service resource agencies and programs. This includes making referrals to community agencies, medical providers, and Regional Support Teams to enhance the overall health and well-being of the individuals served. The role also requires ongoing monitoring of ISPs to ensure that services are meeting the needs of individuals, along with maintaining accurate documentation in the electronic health record system. The position demands a proactive approach to crisis situations, providing basic counseling, coordinating vocational training programs, and ensuring that all necessary residential and institutional placements are managed effectively. The case manager will also engage in community awareness initiatives and family education regarding available DD services and resources. Participation in interdisciplinary meetings and continuing education programs is essential to enhance professional knowledge and skills related to developmental disabilities and supportive counseling.

Responsibilities

  • Assessing the service needs of individuals and families, including completing VIDES assessments and SIS assessments.
  • Formulating written Individual Service Plans (ISPs) that are person-centered and address individual strengths, weaknesses, and service needs.
  • Advocating for individuals by linking them to service resource agencies and programs, including REACH crisis services and managed care organizations.
  • Making referrals to community agencies, medical providers, and Regional Support Teams to improve individual health.
  • Monitoring and reviewing ISPs to ensure services are intact and meeting individual needs, including maintaining accurate record-keeping in electronic health records.
  • Providing basic counseling, including supportive counseling, family assessments, and crisis intervention.
  • Coordinating referrals to vocational training programs and residential placements within the community.
  • Conducting intakes for individuals in need of DD services and promoting community awareness regarding available resources.
  • Participating in agency staffings and interdisciplinary meetings with other agencies or community resources.
  • Responding to crisis situations involving individuals with developmental disabilities and providing timely documentation for admissions, treatment, and discharges.

Requirements

  • Bachelor's degree in a behavioral health-related area preferred.
  • Must possess or obtain the highest level of licensure appropriate to degree.
  • Valid driver's license is required.
  • Experience in mental health and/or intellectual disabilities preferred.
  • Strong verbal and written communication skills are essential.
  • Ability to maintain composure in difficult situations and safely operate a motor vehicle.
