About the position

The Case Manager II at the University of Maryland Medical Center is responsible for providing care coordination and discharge planning for an assigned patient caseload. This role involves assessing individual patient needs, implementing care plans, and monitoring outcomes across the continuum of care, all under the supervision of the Manager of Case Management.

Responsibilities

  • Assess assigned patients and integrate psychosocial assessments within the multidisciplinary team.
  • Collaborate with the multidisciplinary team to determine potential and actual risks to recovery and formulate plans for appropriate levels of care upon disposition from the acute care setting.
  • Develop and coordinate the implementation of Discharge Plan A and alternative Plan B, including documentation in the medical record.
  • Facilitate multidisciplinary collaboration with physicians, nursing, social work, and multiple disciplines, departments, payers, and agencies to eliminate barriers to efficient delivery of care.
  • Facilitate timely transition plans.

Requirements

  • Licensure as a Registered Nurse or other equivalent healthcare license in the state of Maryland.
  • A minimum of 12 months of experience as a case manager.
  • Knowledge of managed care, discharge planning, and case management.
  • Highly effective verbal and written communication skills.
  • Strong communication skills, self-confidence, and experience dealing with physicians.
  • Excellent analytical and team-building skills, with the ability to prioritize and work independently.
  • Ability to work collaboratively with other disciplines.
  • Ability to work with Hospital/Case Management related software programs.
  • Knowledge of principles of growth and development over the lifespan and ability to assess data reflective of the patient's requirements.
