About the position

The Case Manager II, Behavioral Health provides direct patient education and connection to counseling and other community resources and services to individuals and groups as needed. This role involves case management services to patients enrolled in behavioral health services, supporting treatment and ongoing care. The position requires collaboration with patients, internal and external team members, and community resources to enhance patient engagement and recovery.

Responsibilities

  • Provides case management services to patients enrolled with behavioral health services.
  • Assists patients with making referrals to and obtaining appropriate health and human services.
  • Ensures engagement with patients through follow-ups when appointments are missed.
  • Applies strengths-based and trauma-informed approaches in collaboration with patients and team members.
  • Implements motivational interviewing techniques to engage patients in decision-making.
  • Collaborates with community resources to address environmental factors impacting mental health.
  • Enhances education for patients, families, and communities to support engagement and self-management.
  • Coordinates assistance and resources for medical providers treating behavioral health patients.
  • Monitors patient access to treatment and involvement with providers.
  • Follows up with patients to ensure timely follow-through on referrals.
  • Monitors patients for changes in clinical symptoms and consults with the treatment team.
  • Works closely with medical providers and counselors to provide ongoing services.
  • Ensures accurate billing for services to AHCCCS payors.
  • Assists with completion of intakes and assessments, including treatment and service plans.
  • Provides services under clinical supervision to meet billing and documentation requirements.
  • Ensures timely completion of documentation in EHR.
  • Keeps treatment and service plans up to date, including annual updates.
  • Assists with care coordination for patients in residential treatment facilities.
  • Ensures compliance with trainings and guidelines from CCHCI and AHCCCS.
  • Participates in assigned meetings, staff development, and training.
  • Tracks and reports on patients receiving services.
  • Provides excellent customer service to patients and community agencies.
  • Assists with community engagement to support CCHCI services.
  • Performs other related duties as assigned.

Requirements

  • Bachelor's degree in behavioral health field.
  • Two years of experience in providing direct case management services in a behavioral health setting.
  • Valid Fingerprint Clearance Card.
  • Driver's License and Proof of Insurance may be required.

Nice-to-haves

  • Master's Degree in the field of behavioral health.
  • One year of direct case manager experience related to mental health diagnoses.
  • One year of experience in providing patient care in a behavioral health setting.
