About The Position
Diakon is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Child, Family, and Community Ministries and Diakon Affordable Housing. The position involves providing a full range of in-depth social work services, including case management to client populations in accordance with state regulations and agency policies. The role requires employing a range of social work interventions with client families to facilitate the accomplishments of goals listed in the Service Plan. Responsibilities include making initial case assessments and ongoing assessments, as well as providing case management and face-to-face services as described in the Service Plan to meet stated client goals.
