About the position

The City of Fremont's Human Services Department is seeking a Case Manager (Information and Assistance) to support individuals and families in the community. This role involves managing a caseload, conducting comprehensive assessments, developing case plans, and coordinating services to promote self-sufficiency and family stabilization. The Case Manager will also provide counseling, monitor service effectiveness, and engage in community outreach and education.

Responsibilities

  • Manage a caseload of clients, ensuring required volume is met.
  • Conduct comprehensive client assessments to gather relevant information for case planning.
  • Develop support systems to meet client needs and promote independent living.
  • Determine the need for inter-agency and family conferences and conduct them as necessary.
  • Provide supportive counseling and advocacy for clients.
  • Monitor and verify services provided to clients on a monthly basis, assessing quality and effectiveness.
  • Interact with citizens and city departments via phone and in public presentations.
  • Prepare and present training and educational programs and publicity materials.
  • Prepare written and statistical technical reports and case notes.
  • Develop and implement new processes and procedures for case management.
  • Assist in supervising intern training and conduct intake assessments for other social service programs.

Requirements

  • Master's Degree in social work, psychology, counseling, sociology, nursing, gerontology, or a related field and one year of case work experience, or a Bachelor's Degree in the same fields with three years of social service experience, including one year with children, youths, or senior citizens.
  • Knowledge of social work principles and practices, interviewing techniques, and counseling modalities.
  • Ability to conduct assessments, develop case treatment plans, and maintain effective casework relationships with clients.
