About the position

The Case Manager for Inpatient Acquired Brain Injury at Shepherd Center plays a crucial role in facilitating patient care throughout the continuum of care. This position involves working within an interdisciplinary team to ensure effective care coordination, resource utilization, and clinical documentation, while advocating for patients and their families. The Case Manager is responsible for assessing, planning, implementing, and evaluating patient care plans, ensuring high standards of care and patient satisfaction.

Responsibilities

  • Support advocacy for patients and their families by promoting high standards of patient care.
  • Work closely with clinic physicians to address patient issues such as therapy goals and financial status.
  • Develop a mutually acceptable plan of care with patients and families to maximize their potential.
  • Bridge communication between providers, patients, families, and outside agencies to ensure high-quality care.
  • Coordinate physician orders to outside suppliers, including home health and durable medical equipment.
  • Collaborate with supervisory staff to ensure quality patient care within Shepherd Center's policies.
  • Promote a multidisciplinary approach to coordinate care and plan operations among disciplines.
  • Act as a resource for case manager associates and assist with problem-solving.
  • Manage requests for medication assistance and related documentation.
  • Provide education, guidance, and support post-discharge focusing on medical, financial, and psychosocial needs.
  • Complete thorough and accurate documentation in electronic records.
  • Coordinate language services for interpretation needs in the clinic.
  • Maintain appropriate certification and knowledge of relevant regulations and standards.

Requirements

  • Associate or Bachelor's degree in allied health, nursing, or related field, or appropriate state licensure.
  • Certification in Case Management within two years of position start date.
  • Minimum of two years broad clinical experience, preferably in care coordination and case management.
  • Effective time management skills.
  • Excellent verbal and written communication skills.
  • Effective decision-making and problem-solving skills.
  • Demonstrated critical thinking skills and ability to anticipate patient discharge needs.
  • Understanding of the interdisciplinary team approach and rehabilitation environment.
  • Moderate to expert computer skills.
  • Working knowledge of financial aspects of third-party payors and reimbursement.
