About the position

The Case Manager/Intake Coordinator at Cherokee Indian Hospital Authority is responsible for managing applications for the Residential Treatment Program and providing case management services to individuals with mental health and substance use disorders. This role involves initial patient orientation, maintaining accurate patient files, and ensuring access to necessary care and services. The position is integral to the Kanvwotiyi Residential Program, focusing on person-centered planning and collaboration with healthcare professionals to facilitate patient care and support.

Responsibilities

  • Provide case management functions including assessment, planning, referral, and monitoring according to CIHA policy.
  • Process applications for the Residential Treatment program, coordinating interviews and scheduling intake appointments.
  • Maintain caseload by identifying new patients and discharging patients as per CIHA policy.
  • Document treatment and significant events in the Electronic Health Record (EHR) as per CIHA standards.
  • Gather and document necessary patient information in the EHR across various domains.
  • Engage patients in the planning process to help them achieve desired outcomes.
  • Link clients to necessary services and supports.
  • Provide timely updates to treatment team members regarding client status and safety.
  • Collaborate with medical staff and other professionals to facilitate patient care and follow-up.
  • Consult with patients and families regarding treatment limitations and ongoing care.
  • Coordinate discharge planning and follow-up care with internal and external providers.
  • Attend clinical staffing to discuss client data and treatment plans.
  • Participate in meetings to improve program and treatment quality.
  • Provide consultation services to various organizations affecting patient care.
  • Support patients in both office and community settings, including home visits.
  • Represent CIHA in team meetings and emergency situations.
  • Participate in community planning to address client care issues.
  • Collect and report data necessary for client care as directed by supervisor.
  • Obtain continuing education through training and workshops.
  • Support Tribal Option Care Managers as needed.

Requirements

  • Bachelor's degree in a human service field.
  • Experience working with substance use and/or mental health disorders.
  • Valid driver's license.
  • Preferred experience working with Native Americans.
  • Two years' experience with the target population preferred.
