About the position

The Case Manager/Intake Specialist will provide essential services to homeless transition-aged youth (18-24 years old) under the direction of the Ruth's Place Manager. This role involves determining client eligibility for services, providing case management, and facilitating outreach and enrollment efforts to support youth in accessing necessary resources and services.

Responsibilities

  • Certify client documentation as prescribed by LASHA regulations and organize client files.
  • Conduct orientations and information sessions for small and large groups.
  • Coordinate presentations and enrollment of participants with mandatory partners and cross-functional teams.
  • Review enrollment packets for completeness and accuracy.
  • Determine accurate participant enrollment into the correct programs.
  • Conduct outreach and marketing efforts to promote Ruth's Place services and programs.
  • Oversee the intake office, providing outreach, resources, intake, and assessment of clients.
  • Attend meetings and trainings to improve skills and knowledge of best practices in Permanent Supportive Housing.
  • Provide exceptional customer service to new and existing consumers.
  • Enter client demographic information into HMIS and Salesforce databases as needed.
  • Maintain contact with partner agencies via email and phone calls.
  • Schedule in-person assessments and coordinate admissions as required.
  • Develop and organize enrollment packets with current and updated forms mandated by LAHSA.
  • Participate in Care Coordination Meetings and community resource events.
  • Provide linkage to youth for resources such as education, employment, and mental health services.
  • Formulate HSPs that identify youth needs and goals to help decrease barriers to housing and employment.

Requirements

  • Knowledge of database management and quality control processes.
  • Reliable and flexible team player with a good work ethic.
  • Ability to work on multiple projects simultaneously and meet deadlines.
  • Demonstrated knowledge of community resources.
  • Highly organized and able to work within multiple databases.
  • Valid driver's license and car insurance as required by law.
  • Bilingual in English/Spanish preferred but not required.

Nice-to-haves

  • Experience in quality control.
  • Familiarity with the homeless TAY population.
