About the position

The Case Manager for Intensive Case Management Services (ICMS) at Venice Community Housing (VCH) provides essential support to individuals who have experienced chronic homelessness. This role focuses on delivering intensive case management services in a permanent supportive housing environment, utilizing evidence-based practices to help tenants stabilize in housing, improve their quality of life, and integrate into the community. The Case Manager collaborates with various community service providers to ensure comprehensive support for clients, addressing their unique needs and goals.

Responsibilities

  • Conduct intake and enrollment activities with eligible clients, assisting with documentation and project intake forms.
  • Assist clients with move-in processes to permanent housing, including becoming document ready and coordinating move-in logistics.
  • Perform initial comprehensive biopsychosocial assessments with quarterly updates.
  • Create and review client-driven, person-centered Individual Service Plans (ISP) with tenants.
  • Provide field-based Intensive Case Management services focusing on independent living skills, housing stabilization, and community integration.
  • Mediate and advocate for tenants to help them obtain housing, health care benefits, and other necessary services.
  • Maintain up-to-date tenant files with all required documentation.

Requirements

  • Bachelor's degree in social science or related field, or equivalent work/lived experience preferred.
  • One to two years of experience in case management or a related field.
  • Experience working with homeless individuals with substance abuse, mental health issues, and/or chronic health conditions.
  • Effective communication skills with diverse groups, especially those who have experienced trauma.
  • Valid California Driver License and proof of insurance.

Nice-to-haves

  • Bilingual Spanish-English preferred, but not required.
  • Experience and knowledge of Westside community and social services.
