About the position

The Case Manager - IOP SUD at SSTAR is responsible for providing comprehensive case management services for the Intensive Outpatient Program (IOP) focused on substance use disorders (SUD). This role involves facilitating client treatment plans, coordinating care, and ensuring clients receive the necessary support and resources for their recovery journey. The position emphasizes a holistic approach to health, addressing mental, physical, and spiritual well-being, and requires collaboration with various healthcare providers.

Responsibilities

• Obtain initial insurance authorizations and extensions for insurance authorizations.
• Check daily attendance and maintain admission log, distributing weekly census reports.
• Facilitate group sessions and document all collateral contacts and services in the Electronic Health Record (EHR).
• Meet with clients to develop and review treatment plans.
• Link clients to appropriate services such as primary care, transportation, and psychiatric support.
• Arrange aftercare appointments when discharge is planned.
• Conduct outreach via phone or letter for clients who drop out of the program.
• Participate in team meetings and program supervision.
• Market the program and participate in the ongoing development of the IOP curriculum.
• Provide one-on-one counseling and complete discharge summaries.

Requirements

• Bachelor's degree preferred in Social Work or a related field.
• 2-4 years of experience in a relevant role.
• Valid RSN, CADC, or LADCII certification required.
• Knowledge of substance use disorders and mental health counseling.
• Prior knowledge of health insurance authorizations.
