About the position

The Case Manager for Kinship Care is a part-time position based in Jonesboro, GA, aimed at providing essential case management services, information, referrals, and advocacy for grandparents and other relative caregivers. This role is crucial for supporting older adults, caregivers, and families by addressing their unique needs. The Case Manager will be responsible for accepting and recording referrals, conducting home visits, and completing initial assessments to determine the eligibility of participants through a specified intake process. In this position, the Case Manager will review and analyze information compiled on participants to identify their needs and assist in developing care/service plans that outline the specific services to be provided. The role also involves identifying and mobilizing resources, developing a resource file to supplement services, and collaborating with other service providers when feasible. Maintaining accurate and current files, records, and reports on each participant is essential, including documentation of any changes in the participant's status or services provided. The Case Manager will also visit hospitals, the Housing Authority, the Social Security Department, and other relevant organizations to gather information and identify resources for future referrals and follow-up activities. Participation in staff meetings, workshops, seminars, and training sessions is expected to enhance social services skills. Additionally, the Case Manager will prepare reports as needed, uphold participants' rights, and work closely with the department's Information and Referral Specialist to complete applications for energy assistance, surplus commodities, and Medicaid. Other duties may be assigned as necessary.

Responsibilities

  • Visits hospitals, Housing Authority, Social Security Department, and other agencies to develop information and identify resources for referrals.
  • Attends and participates in staff meetings, workshops, seminars, and training sessions to enhance social services skills.
  • Prepares and submits reports as needed or required.
  • Maintains participant's rights.
  • Completes applications for energy assistance, surplus commodities, and Medicaid.
  • Makes appropriate referrals for CCSP, volunteer services, financial, and other needs.

Requirements

  • Bachelor's degree in Social Work or a related field.
  • In lieu of a degree, two years of college and two years of experience in human services delivery, preferably in geriatrics.
  • Valid Georgia Driver's License and reliable transportation.
