About the position

The Housing Retention Case Manager at La Posada is responsible for supporting clients in accessing essential services to achieve and maintain stable housing and employment. This role involves conducting assessments, developing personalized service plans, and providing ongoing case management support. The position emphasizes strengths-based approaches and evidence-based practices to empower clients, particularly those facing challenges related to substance use and homelessness.

Responsibilities

  • Support clients in accessing services and resources.
  • Conduct case management assessments and develop personalized service plans.
  • Link clients to employment services and address health and wellness objectives.
  • Provide ongoing outreach and case management to clients in the program.
  • Implement evidence-based practices such as Motivational Interviewing and Trauma-Informed Care.
  • Educate tenants about substance use and assist in obtaining treatment services.
  • Collaborate with tenants to explore ways to reduce substance use.
  • Conduct interviews and consult with professionals to evaluate client conditions.
  • Develop tailored treatment and rehabilitation programs for clients.
  • Provide individual counseling to support clients in overcoming dependency issues.
  • Monitor client progress and prepare reports to prevent substance abuse.
  • Assist tenants in setting personal goals related to substance use.
  • Provide relapse prevention support after treatment completion.
  • Address smoking issues and assist with cessation resources.
  • Ensure a safe environment for clients and provide crisis intervention when necessary.
  • Transport clients for appointments as required.
  • Maintain accurate and timely documentation in the ETO and EHR systems.

Requirements

  • High School Diploma or equivalent required; Associate's Degree in a related field preferred.
  • Minimum of 2 years of experience in social services, particularly with homeless populations.
  • Experience in case management practices and counseling techniques.
  • Excellent communication skills for effective interaction with clients and stakeholders.
  • Crisis intervention skills to handle and de-escalate conflicts.
  • Empathy and compassion for individuals experiencing homelessness.
  • Ability to work collaboratively in a team environment.
  • Proficiency in MS Office (Word, Excel) and Google applications.

Nice-to-haves

  • Experience with substance abuse treatment programs.
  • Knowledge of community resources for homeless individuals.
  • Bilingual skills in English and Spanish.
