About the position

The Case Manager LPN at WellMed, part of the Optum family, plays a crucial role in supporting patients with stable chronic health conditions as they navigate the healthcare system. This position is designed for individuals who are passionate about helping others achieve better health outcomes. The Healthcare Coordinator acts as an educator, resource, and advocate for patients and their families, ensuring they receive the highest level of medical care. The role involves collaboration with multidisciplinary care teams, including physicians, nurses, pharmacists, and social workers, to provide comprehensive support to patients. In this position, the Healthcare Coordinator will work in a less structured, self-directed environment, performing nursing duties within the scope of a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license. Responsibilities include identifying patients with chronic disease diagnoses, performing focused assessments, conducting medication reconciliations, and providing patient education using standardized materials. The Coordinator will also assist in the transition of patients from inpatient to outpatient settings, ensuring that all post-discharge needs are met, including prescriptions and transportation. The role requires accurate and timely documentation in electronic medical records, participation in daily huddles and Patient Care Coordination meetings, and preparation of reports for weekly meetings. The Healthcare Coordinator is expected to maintain continued competence in nursing practice and stay updated on current evidence-based practices. Responsibilities

  • Identify patients with chronic disease diagnoses in collaboration with providers and clinic staff.
  • Perform focused assessments of chronic care conditions.
  • Conduct medication reconciliation for patients.
  • Utilize motivational interviewing techniques and assist in self-management goal setting.
  • Provide patient education using standardized, approved educational materials.
  • Create referrals to appropriate agencies and resources for patients.
  • Support the transition of patients from inpatient to outpatient settings by assessing transitional needs.
  • Assist with post-discharge needs such as prescriptions, transportation, and appointments.
  • Notify providers and/or supervising RN of changes to patient's health condition.
  • Document patient interactions accurately and timely in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare accurate and timely reports for weekly meetings.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.

Requirements

  • Active and unrestricted TX licensure as a Licensed Practical/Vocational Nurse.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Excellent verbal and written communication skills.
  • Solid interpersonal skills for effective interaction with multidisciplinary teams.
  • Proficient computer skills for working with electronic medical records.
  • Excellent organizational and prioritization skills.
