Responsibilities

• Provide case management services to members in an acute care setting or SNF.
• Follow identified members through the continuum of care until discharge is appropriate.
• Communicate professionally with healthcare physicians and hospital staff.
• Respond to inquiries from members, vendors, and regulatory agencies with excellent customer service.
• Perform clinical nursing skills including assessment, planning, evaluations, and outcomes.
• Apply the nursing process effectively in case management.
• Demonstrate problem-solving and decision-making skills in a clinical context.
• Prioritize tasks and manage multiple demands in a timely manner.

Requirements

• Licensed LVN or RN required.
• Demonstrated independent work ethic and decision-making skills.
• Excellent communication skills, both written and verbal, in English.
• Strong organizational skills and ability to prioritize tasks.
• Computer literacy, including proficiency in Microsoft Excel and Word.
• Ability to perform clinical nursing skills and apply the nursing process.
