About the position

The Case Manager will provide essential case management services in partnership with the Sterling Heights Police Department, focusing on individuals in need of behavioral health support. This role involves assessing clients, connecting them to necessary resources, and ensuring they receive comprehensive care to avoid further involvement in the criminal justice system. The position includes an on-call component and collaboration with law enforcement and social service providers to facilitate ongoing support for participants.

Responsibilities

  • Conduct comprehensive assessments for individuals referred by Sterling Heights personnel.
  • Provide case management services aimed at connecting participants to healthcare, mental health, and human services resources.
  • Collaborate with peer recovery coaches to ensure participants have ongoing support and access to treatment.
  • Facilitate warm hand-offs to various services, including health insurance enrollment, housing support, food assistance, and healthcare.
  • Maintain regular communication with the Sterling Heights Police Department and other community stakeholders to monitor the progress of referred individuals.
  • Regularly meet with the Program Coordinator and clinical supervisors to discuss case progress and service delivery improvements.
  • Keep detailed case notes and data for ongoing evaluation of service effectiveness, ensuring compliance with FAN's outcomes management system.
  • Participate in crisis interventions as needed, including connecting individuals with immediate care and support.
  • Attend ongoing training sessions to enhance case management skills, including motivational interviewing and crisis response strategies.
  • Assist in the collection of data for outcome reporting, ensuring the program meets its metrics for success.
  • Ensure privacy and confidentiality of all client information, adhering to HIPAA and FAN's confidentiality policies.
  • Participate in on-call shifts to provide 24/7 support as needed.
  • Work collaboratively with a partner Case Manager on two separate shifts, ensuring continuity of care for program participants.

Requirements

  • Bachelor's degree in Social Work, Criminal Justice Counseling, Psychology, or a related field required or extensive professional/lived experience.
  • 2+ years of experience in case management, preferably within a behavioral health, criminal justice, or social services setting.
  • Strong understanding of mental health and substance use disorder services.
  • Experience working with law enforcement, courts, or community health programs is highly desirable.
  • Excellent communication and interpersonal skills, with the ability to work in a collaborative team environment.
  • Proficiency in data management and reporting.
  • Ability to handle multiple cases with varying levels of complexity and need.
  • Valid driver's license and reliable transportation.
