About the position

The Case Manager at MaineHealth is responsible for enrolling participants into the program and assessing their emotional, social, and environmental needs. This role involves providing casework services, including information and referral, psychosocial support, and advocacy, while collaborating with healthcare and social service providers to optimize participants' health. Additionally, the Case Manager assists with administrative monitoring of the program, including preparing statistical reports related to program activities.

Responsibilities

• Enroll participants into the program.
• Assess the emotional, social, and environmental needs of participants.
• Provide casework services including information and referral, psychosocial support, and advocacy.
• Collaborate with healthcare, mental health, and social service providers to optimize participant health.
• Assist with administrative monitoring of the program, including preparation of statistical reports.

Requirements

• Bachelor's degree preferred or equivalent relevant experience in a healthcare environment.
• Valid Maine driver's license and reliable vehicle may be required for off-site travel.
• Approximately two years of experience in a community health setting to demonstrate knowledge of community resources.
• High level of analytical skills and good judgment in assessing and planning for participant needs.
• Competency in developing and presenting information to groups of healthcare and service providers.
• Ability to operate software systems that support program activities and extract statistical reports.
• Strong customer service, teamwork, resource knowledge, and advocacy skills.
