About the position

The Case Manager Mental Health Professional - Intake position at the Association for Individual Development (AID) focuses on empowering individuals with mental health challenges. This role involves conducting client intakes, providing case management, and coordinating outpatient therapy services. The position is integral to supporting clients in various programs, including supported living and outpatient services, while advocating for their rights and ensuring quality care.

Responsibilities

  • Complete Intake/IM+CANS for new clients in Region 5.
  • Provide set up through Zoom for psychiatry services in DeKalb County Jail.
  • Provide/Arrange outpatient therapy-individual, as assigned.
  • Provide case management to a small supported living caseload, as assigned.
  • Provision of assessment, counseling, skills training, and case management services to clients diagnosed with persistent mental illness disorders.
  • Conduct new client intakes including IM+CANS and LOCUS Assessments as assigned.
  • Provide documentation of services in accordance with DHS and CARF.
  • Advocate for and maintain rights of clients served.
  • Assign clients to case managers' caseloads in coordination with the program manager.

Requirements

  • Bachelor's degree in human services or a related field (required).
  • Experience working in social services (required).
  • A valid Illinois driver's license and automobile insurance.
