About the position

The Mental Health Case Manager provides outpatient mental health services tailored to meet the needs of designated consumers. This role involves evaluating consumer needs, delivering therapeutic interventions, and developing treatment plans for adults with persistent and severe mental illness. The position requires active participation in therapeutic groups and collaboration with community agencies and healthcare providers.

Responsibilities

• Develop and implement treatment plans for adults with persistent and severe mental illness.
• Participate and lead therapeutic groups and activities.
• Assume overall case management responsibilities for assigned consumers.
• Coordinate and liaise with other community agencies and healthcare systems.
• Maintain appropriate patient records according to established guidelines.
• Drive consumers to and from programs in 12 passenger vans on occasion.

Requirements

• Bachelor's degree in a relevant field.
• Experience in behavioral health.
• Valid driver's license.
