About the position

We are seeking a dedicated and compassionate Case Manager to join our women's transitional home. The ideal candidate will play a crucial role in coordinating women's services and ensuring that individuals receive the necessary support throughout their treatment journey. This position requires a strong understanding of medical records, utilization management, and discharge planning.

Responsibilities

• Assess patient needs and develop individualized care plans that address both medical and psychosocial aspects of health.
• Coordinate with healthcare providers, patients, and families to facilitate effective communication and collaboration throughout the treatment process.
• Conduct thorough reviews of medical records to ensure compliance with care standards and regulations.
• Implement discharge planning processes to ensure smooth transitions from facility to home or other care settings.
• Provide education and resources to patients regarding their conditions, treatment options, and available support services.
• Utilize knowledge of physiology to inform care decisions and interventions.
• Engage in addiction counseling as needed to support patients facing substance use challenges.
• Monitor patient progress and adjust care plans as necessary based on ongoing assessments.

Requirements

• Previous experience in case management or a related field is preferred, particularly within a clinical or hospital setting.
• Familiarity with utilization management principles and practices is highly desirable.
• Strong understanding of medical records management and documentation standards.
• Excellent communication skills, both verbal and written, with the ability to interact effectively with diverse populations.
• Ability to work collaboratively within a multidisciplinary team environment.
• Knowledge of community resources available for patient support is beneficial.
