About the position

The Case Manager at David & Margaret Home Inc. is responsible for providing quality case management services to vulnerable populations, particularly in crisis situations. This role involves conducting intake interviews, assessing family relationships, and coordinating reunification efforts for youth. The Case Manager must ensure compliance with state and federal standards while maintaining effective communication with families and other stakeholders.

Responsibilities

  • Conduct initial intake interviews of unaccompanied children (UCs) to gather familial and sponsorship information.
  • Assess family members and sponsors to verify information received from minors within 24-48 hours of admission.
  • Determine options for youth within 48-72 hours and document the process for reunification.
  • Provide weekly updates to youth and telephonic updates to family members or sponsors, documenting interactions in the UAC Portal.
  • Ensure all information is entered into the UAC portal within the ORR timeline.
  • Submit completed cases for internal review in a timely manner according to ORR guidelines.
  • Facilitate three weekly telephonic contacts with family of origin, primary caregivers, or sponsors.
  • Coordinate incoming calls between minors and approved family members or caregivers.
  • Facilitate attorney-client contact as requested by UCs or attorneys.
  • Coordinate weekly treatment team meetings with shelter representatives from all departments.
  • Attend case staffing and pre-staffing meetings with funding source representatives.
  • Maintain physical and web-based client files.
  • Drive clients to facilitate program services, including court appointments and attorney visits.
  • Coordinate travel arrangements for UCs to their sponsors' locations.
  • Perform all other duties as assigned.

Requirements

  • Bachelor's degree in social work, education, psychology, sociology, or other behavioral science equivalent.
  • Excellent organizational, verbal, written, and interpersonal communication skills in English and Spanish.
  • A minimum of two years of experience providing case management services to vulnerable populations, preferably immigrant or refugee children.
  • Capacity to deliver trauma-informed services and navigate care for children with extensive trauma histories.
  • Ability to have a flexible schedule to accommodate occasional after-hours services, including evenings and weekends.
  • Valid U.S. driver's license, valid auto insurance, a clean driving record, and personal transportation.
  • Ability to successfully pass a criminal background check.
