About the position

The Case Manager position at the Crisis Stabilization Facility focuses on developing clinically based treatment relationships with individuals diagnosed with serious and persistent mental illness. The role involves assisting clients in integrating into the community through structured recovery plans, which include coordinating psychiatrist visits, medications, housing, and other essential services. This position operates overnight, providing support in a therapeutic environment, and emphasizes teamwork and ongoing training.

Responsibilities

• Develop clinically based treatment relationships with clients diagnosed with serious mental illness.
• Assist clients in integrating into the community using structured recovery plans.
• Coordinate psychiatrist visits, medications, housing, and substance abuse treatment for clients.
• Communicate with stakeholders in the community and complete vital documentation.
• Host skill-building and recreational groups onsite to aid in client recovery.

Requirements

• High school diploma or GED required.
• Bachelor's degree in Psychology, Social Work, Health, Nursing, Counseling, or similar preferred.
• 6-12 months supervised experience with populations such as severely mentally ill, developmentally disabled, elderly, or children preferred.
• Valid driver's license and access to a vehicle preferred.
