About the position
The Case Manager position is a remote, contracted role focused on providing support and guidance to K-12 students and their families within a public school district in California. This position offers flexibility for full or part-time hours and requires a dedicated professional to manage a caseload effectively while collaborating with educators and parents to implement intervention strategies.
Responsibilities
- Manage a caseload of K-12 students, providing individualized support and case management services.
- Collaborate with educators, parents, and other professionals to develop and implement effective intervention strategies.
- Conduct assessments and develop personalized education plans (IEPs) for students.
- Monitor student progress and adjust strategies as needed to ensure success.
- Maintain accurate and up-to-date documentation of services provided and student progress.
- Participate in virtual meetings and trainings to stay current with best practices and policies.
Requirements
- Master's degree in Social Work, Education, or related field.
- Experience in case management or working with K-12 students.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a remote caseload effectively.
- Familiarity with virtual platforms and technology used in remote education is a plus.
- Current licensure or certification relevant to case management in California.
