About the position

The Case Manager for the Pathway of Hope program is responsible for coordinating social services activities, providing direct assistance and referrals to clients in need, and assessing families with minor children for eligibility in the program. The role emphasizes a strength-based case management model to help families break the cycle of crisis and improve their life trajectories.

Responsibilities

  • Conduct regular outreach to social services sites to identify eligibility requirements.
  • Engage and build rapport with the target population.
  • Conduct screening interviews with potential applicants according to POH eligibility requirements.
  • Present potential participants to the POH team for evaluation.
  • Provide appropriate referrals for individuals not eligible for POH services.
  • Perform intake that includes required documentation for admission and referral needs.
  • Conduct a written assessment on all participants within 72 hours of contact.
  • Develop a case plan with participants focusing on housing stability and self-determination.
  • Schedule regular meetings with participants to review goals and objectives.
  • Conduct home visits as determined by the case plan.
  • Provide information and referral services as needed.
  • Assist participants in connecting to community resources.
  • Provide advocacy services as needed, including court involvement and conflict resolution.
  • Provide financial assistance in accordance with program policies.
  • Coordinate case management efforts with all staff and contracted services.
  • Develop a discharge plan with participants addressing stability and self-determination.
  • Assist participants in developing a crisis plan and be available for emergencies.
  • Conduct life skills and budgeting classes.
  • Maintain case files and comprehensive case notes for each participant.
  • Complete required documentation of educational groups and prepare case records for storage.
  • Ensure client confidentiality in accordance with established procedures.
  • Maintain comprehensive demographic data on all participants.
  • Submit monthly service statistics to the program manager.
  • Maintain accurate records of financial assistance provided to participants.
  • Attend agency and community meetings as requested.
  • Maintain working relationships with community agencies for comprehensive services.
  • Provide guidance to community volunteers working with the program.
  • Promote the spiritual dimension of Salvation Army social services programs.
  • Track and report unmet needs of participants and their families.
  • Document and report effectiveness of service delivery using feedback.
  • Ensure quality of local data and reporting of information for POH outcomes.
  • Ensure accuracy of data entry into the SIMS database.
  • Participate in program evaluation activities and assist in annual evaluations.
  • Attend training sessions and workshops as required.

Requirements

  • Degree in human service area; prefer bachelor's degree from an accredited college or university.
  • Minimum of two years of case management experience in a comparable social service program.
  • Experience in strength-based case management is preferred.
  • Strong interest in community outreach and capacity development.
  • Ability to work with diverse racial, ethnic, and socio-economic backgrounds.
  • Ability to collaborate on complex social issues within families and communities.
  • Creative problem-solving skills and ability to make appropriate judgments.
  • Excellent oral and written communication skills.
  • Clear criminal record.
