About the position

The Case Manager/Patient Care Coordinator at the Institutes of Health is responsible for managing patient referrals, coordinating care for first responder patients, and ensuring effective communication among various stakeholders. This role is crucial in facilitating patient intake and treatment processes within a multidisciplinary clinic focused on chronic pain and prescription drug abuse.

Responsibilities

  • Receives and processes referrals for first responder patients from various sources.
  • Requests for authorization submission and tracking.
  • Monitors the status of pending reports.
  • Accurately enters patient information into the EMR system.
  • Maintains regular communication with medical and psychological providers, referral sources, insurance adjusters, and the internal marketing team regarding the patient's treatment progress.
  • Contacts patients to schedule their first appointment, ensuring all necessary information is gathered.
  • Prepares and manages new patient intake packets through the patient portal, verifying that all required documents are completed thoroughly before the first appointment.
  • Reviews and adjusts provider schedules to accommodate new patient appointments at the soonest available time.
  • Facilitates coordination of entry into Intensive Outpatient Programs (IOP) by working with patients, providers, case managers, and inpatient treatment facilities.
  • Performs any and all other duties and responsibilities as assigned.

Requirements
