About the position

The Case Manager position at ClearSky Rehabilitation Hospital is a part-time role focused on providing exceptional patient-centered care. The Case Manager collaborates with physicians and multidisciplinary teams to develop and monitor care plans for patients, ensuring they progress towards desired outcomes. This role is essential in identifying and resolving barriers to effective patient care and actively participating in the discharge planning process.

Responsibilities

• Communicates, collaborates, and coordinates with team members to provide quality patient care and ensure positive patient outcomes.
• Facilitates communication during interdisciplinary team conferences.
• Communicates appropriate information for timely updates and authorizations with payors.
• Responsible for accreditation standards and adheres to all standards set forth by the State and accrediting agencies of TJC and CMS.
• Documents communication and coordination of patient activities, medical necessity, and post-discharge equipment and support needs in the healthcare record.
• Supports and facilitates length of stay management.
• Conducts utilization reviews.
• Provides an environment conducive to safety for patients, visitors, and staff.
• Assesses the risks for safety and implements appropriate precautions.
• Complies with appropriate and approved safety and Infection Prevention standards.

Requirements

• Current state clinical licensure as an RN, LPN, PT, OT, SLP, or LSW is required.
• Two (2) years' recent relevant experience is required.
• Experience in an Acute care setting is preferred.
• Bachelor's degree preferred.
• Must maintain a valid, acceptable driving record, current driver's license, and insurability.
