About the position

The Case Manager at MercyOne Clive Rehabilitation Hospital plays a crucial role in the interdisciplinary team, focusing on the care and rehabilitation of patients in an inpatient setting. This position is responsible for coordinating patient care, ensuring effective communication among team members, and facilitating the development and monitoring of treatment plans to achieve optimal patient outcomes. The role emphasizes collaboration, documentation, and patient advocacy within a supportive and mission-driven environment.

Responsibilities

  • Completes departmental orientation, initial and annual competencies.
  • Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care, and develop solutions.
  • Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedules family conferences and communicates with caregivers to keep them informed of patient progress and rehabilitation plans.
  • Coordinates weekly patient care team conferences to develop and refine treatment plans.
  • Reviews the patient's assigned CMG and identifies any potential missed comorbid conditions, communicating findings to the HIM team.
  • Performs other duties as assigned.

Requirements

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
  • Minimum of 2 years of social work or case management experience in an inpatient setting, preferably in an acute/rehabilitation hospital.
  • Effective oral and written communication skills in English; additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.

Nice-to-haves

  • Experience in acute/rehabilitation hospital settings.
  • Certification in Case Management or Rehabilitation Nursing.
