About the position

The Case Manager position at MercyOne Clive Rehabilitation Hospital is a PRN role focused on providing comprehensive care coordination for patients in an inpatient rehabilitation setting. The role involves collaborating with an interdisciplinary team to ensure optimal patient outcomes and effective discharge planning, while also supporting the hospital's mission to deliver superior patient care.

Responsibilities

  • Completes departmental orientation, initial and annual competencies.
  • Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care, and develop solutions.
  • Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedules family conferences and communicates with caregivers to keep them informed of patient progress and discharge plans.
  • Coordinates weekly patient care team conferences to develop and refine treatment plans.
  • Reviews the patient's assigned CMG and identifies any potential missed comorbid conditions, communicating findings to the HIM team.
  • Performs other duties as assigned.

Requirements

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
  • Minimum of 2 years of social work or case management experience in an inpatient setting, preferably in an acute/rehabilitation hospital.
  • Effective oral and written communication skills in English; additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
