About the position

As a Case Manager in Quality Assurance at HOPE Community Services, Inc., you will play a crucial role in maintaining and enhancing service quality across various programs. This position involves reviewing clinical documentation, collaborating with staff to ensure compliance with regulations, and identifying areas for improvement. Responsibilities

  • Review clinical documentation to ensure compliance with quality standards.
  • Collaborate with staff to identify areas for improvement in service delivery.
  • Monitor trends and deliver training to staff on quality assurance practices.
  • Maintain a proactive approach to quality assurance in assigned programs.
  • Support the Director of Quality and Integration in enhancing service quality.

Requirements

  • Ability to obtain and maintain Case Management II (CM II) certification.
  • Detail-oriented with strong analytical skills.
  • Strong verbal and written communication skills.
  • Empathy and a commitment to maintaining a high standard of care.
  • Strong organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
