About the position
The Case Manager at Four Oaks Family and Children's Services is responsible for the planning and treatment of children and families, ensuring their well-being and development through tailored care plans. This role involves collaboration with clients, families, and referral agencies to create supportive environments and monitor progress effectively.
Responsibilities
- Developing a care plan for each child containing goals and objectives with projected dates of accomplishment
- Involving the clients, referral agency, and family in planning whenever possible
- Developing a specific plan relating to the involvement of the child's parents
- Participating in the documentation of case assessment
- Documenting the implementation of the care plan
- Providing for scheduled in-person conferences with each resident
- Providing a supportive atmosphere for the youth with leadership and guidance to the client
- Coordinating internal and external activities of the youth
- Serving as a liaison with the referring agency
- Providing a mechanism for dealing with day-to-day program operations
- Overseeing and maintaining the general health and well-being of the child
- Overseeing the living activities of the child
- Monitoring and recording behaviors on a daily basis
Requirements
- Bachelor's Degree in social work or a degree in a human services related field plus two years of experience in child welfare
- Master's Degree in Human Services is required
- Prior experience in working with Foster and Adoptive families is required
- Able to be self-driven/reliant and work independently as well as part of a team
- Strong organizational skills
- Ability to show empathy/compassion to build trust-based relationships
- Effective verbal and written communication skills
