About The Position

The Case Manager at Four Oaks Family and Children's Services is responsible for the planning and treatment of children and families, ensuring their well-being and development through tailored care plans. This role involves collaboration with clients, families, and referral agencies to create supportive environments and monitor progress effectively.

Requirements

• Bachelor's Degree in social work or a degree in a human services related field plus two years of experience in child welfare
• Master's Degree in Human Services is required
• Prior experience in working with Foster and Adoptive families is required
• Able to be self-driven/reliant and work independently as well as part of a team
• Strong organizational skills
• Ability to show empathy/compassion to build trust-based relationships
• Effective verbal and written communication skills

Responsibilities

• Developing a care plan for each child containing goals and objectives with projected dates of accomplishment
• Involving the clients, referral agency, and family in planning whenever possible
• Developing a specific plan relating to the involvement of the child's parents
• Participating in the documentation of case assessment
• Documenting the implementation of the care plan
• Providing for scheduled in-person conferences with each resident
• Providing a supportive atmosphere for the youth with leadership and guidance to the client
• Coordinating internal and external activities of the youth
• Serving as a liaison with the referring agency
• Providing a mechanism for dealing with day-to-day program operations
• Overseeing and maintaining the general health and well-being of the child
• Overseeing the living activities of the child
• Monitoring and recording behaviors on a daily basis
