About the position

The Refugee Intensive Services Case Manager will provide holistic and intensive case management to vulnerable refugee clients, focusing on achieving long-term self-sufficiency. This role involves creating self-sufficiency plans, conducting assessments, and ensuring clients are informed about their rights and responsibilities within the program. The Case Manager will collaborate with various partners to provide comprehensive support and respond to crises effectively, all while promoting the mission of Catholic Charities of Northeast Kansas.

Responsibilities

  • Provide case management services for a caseload of refugees and immigrants participating in the PC - Intensive Case Management (ICM) program.
  • Ensure participants are well-versed in their ICM Program rights and responsibilities, and ensure services are well-documented.
  • Adhere to Office of Refugee Resettlement (ORR) and in-house guidelines on service provision and documentation.
  • Apply a strengths-based approach in case management to support vulnerable clients in obtaining self-sufficiency.
  • Receive referrals for case management for ORR-eligible clients and participate in case conference meetings to assure continuity of care.
  • Assess client needs and work with clients to create Self-Sufficiency Plans (SSP).
  • Regularly meet with all clients on the assigned caseload in alignment with ORR policy for the Preferred Communities grant.
  • Collaborate with internal and external partners to ensure clients receive holistic support to reach goals and achieve long-term self-sufficiency and integration.
  • Respond to crises with appropriate resources and in a timely manner to stabilize clients, maintaining safety and ethical boundaries.
  • Support clients in vulnerable situations, including domestic violence, substance abuse, homelessness, and mental health concerns.
  • Provide advocacy for clients and support accessibility to services, including transportation and language access.
  • Conduct home and field visit assignments as needed and in accordance with agency health and safety protocols.
  • Maintain professional conduct with clients as identified in the NASW Code of Ethics and abide by all program policies and procedures.

Requirements

  • Bachelor's Degree in social work or human service field or comparable experience is preferred.
  • Competency in oral, written, and interpersonal communication skills in English; fluency in a second language (Burmese, Karen, Dari, Swahili, etc.) preferred.
  • Ability to work effectively in a multi-cultural and multi-lingual environment.
  • Strong typing and internet skills, as well as the ability to learn Microsoft Outlook, Excel, and various database programs.
  • Ability to work collaboratively with outside agencies and advocate on behalf of clients.
  • Reliability, punctuality, professionalism, and appropriate responses to people in crisis.
  • Valid driver's license, car insurance, a verified safe driving record, and reliable means of transportation for local travel.
