About the position

The Case Manager for the Rhode Island Works program is responsible for providing case management and care coordination services to individuals and families. This role focuses on ensuring compliance with program requirements, assisting clients in overcoming barriers to progress, and delivering support through comprehensive assessments and service planning. The ideal candidate will possess strong interpersonal skills, a caring nature, and the ability to understand the impacts of trauma and oppression on clients.

Responsibilities

  • Conduct comprehensive assessments on assigned clients, including risk assessments related to basic human needs, vocational factors, and mental health/substance use.
  • Develop service plans with clients in accordance with program provisions.
  • Provide community and home visitation to clients on caseload.
  • Assist clients with applications for emergency funding, housing, utility assistance, medical, and state benefits.
  • Collaborate with other CAP programs and administration to ensure coordination of services and resources.
  • Maintain productivity levels as determined by the Director, in compliance with program standards.
  • Attend program-related meetings and public relations activities as appropriate.
  • Maintain accurate record keeping for client files in accordance with program standards and accreditation requirements.
  • Utilize computer database systems to maintain client information and complete necessary paperwork.

Requirements

  • Bachelor's degree or a minimum of 2 years related experience preferred.
  • Demonstrated case management skills and experience.
  • Knowledge of emergency intervention methods, family dynamics, and health/mental health/substance abuse systems.
  • Knowledge of community resources.
  • Valid Driver's License and proof of liability insurance.
  • Reliable transportation.

Nice-to-haves

  • Bilingual in Spanish is highly preferred.
  • Willingness to obtain a case management/community health worker certificate if required.
