About the position
The Case Manager RN is responsible for supporting patients with high-risk health conditions in navigating the healthcare system. This role emphasizes patient empowerment through education, resource provision, and advocacy, ensuring a high quality of life for patients and their families. The Case Manager collaborates with multidisciplinary care teams and acts as a resource for clinic staff, performing nursing duties within the scope of an RN license.
Responsibilities
- Identify patients at high risk in collaboration with providers and clinic staff.
- Support longitudinal care of patients with chronic conditions by performing health assessments and medication reconciliation.
- Conduct Motivational Interviewing and assist in self-management goal setting.
- Provide patient education and create referrals to appropriate agencies and resources.
- Support the transition of patients from inpatient to outpatient settings by assessing transitional needs and establishing contingency plans.
Requirements
- Bachelor's degree in Nursing or Associate's degree in Nursing with four or more years of experience.
- Registered Nurse with an active and unrestricted license to practice in the state of employment.
- Current BLS certification.
- Two or more years of experience in a physician's office, clinical, or hospital setting.
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
Nice-to-haves
- Experience related to patient education and/or motivational interviewing skills and self-management goal setting.
- Fluent written and verbal skills in English and Spanish.
