About the position

The Case Manager RN in Care Continuity Services at University Health is responsible for ensuring quality clinical and cost-effective outcomes for patients. This role involves collaborating with patients, their support systems, and the healthcare team to assess needs, develop and implement individualized care plans, monitor patient progress, and advocate for necessary services. The position is full-time, working from home, with a schedule from 6:00 AM to 4:30 PM.

Responsibilities

• Identify patients who may benefit from case management services.
• Collaborate with patients and healthcare teams to assess patient needs.
• Develop, implement, and evaluate individualized plans of care.
• Monitor patient progress throughout the continuum of care.
• Advocate for services to meet specific patient needs.

Requirements

• Bachelor of Science in Nursing or equivalent education and experience.
• Licensed as a registered professional nurse in the State of Missouri.
• Excellent verbal, telephonic, electronic, and written communication skills.
• Ability to read and speak the English language.
• Strong partnership building and interpersonal skills.
• Solid clinical knowledge base in medical-surgical nursing.
• Above-average organizational and prioritization skills.
• Proficient computer skills, including Microsoft Word, Outlook, and Excel.
• Current BCLS certification.
• Minimum of two years' experience in an acute care setting.

Nice-to-haves

• RN experience in hospital utilization review, utilization management, and/or case management.
