About the position

The RN Case Manager position at CVS Health is a field-based role focused on providing comprehensive care management for dual eligible populations enrolled in Medicare and Medicaid. Responsibilities

• Assess, plan, implement, and coordinate all case management activities with members.
• Develop a proactive plan of care to enhance short and long-term outcomes for members.
• Conduct evaluations of member needs using clinical tools and data review.
• Incorporate strategies to reduce risk factors and address complex health and social indicators.
• Perform assessments considering various information sources, including claims data.
• Utilize a holistic approach to determine the need for referrals to clinical resources and interdisciplinary team members.
• Collaborate with supervisors and stakeholders to overcome barriers in achieving healthcare goals.
• Present cases at interdisciplinary case conferences and utilize motivational interviewing skills for member engagement.

Requirements

• Active and unrestricted NY RN licensure
• 2+ years of clinical experience
• Minimum 2+ years in Care Management, discharge planning, or home health care coordination
• Willingness to travel 50-75% for field visits in Brooklyn, NY
• Reliable transportation, active driver's license, and proof of vehicle insurance.

Nice-to-haves

• Previous experience with insurance
• UAS-NY certification preferred
• LTSS experience preferred
• Confidence working independently from home
• Excellent analytical and problem-solving skills
• Effective communication, organizational, and interpersonal skills
• Proficiency in navigating multiple systems and keyboarding
• Demonstrated proficiency with MS Word, Excel, Outlook, and PowerPoint.
