About the position

The Case Manager at CVS Health is responsible for delivering quality healthcare services to members through comprehensive assessments and care planning. This remote position involves significant field travel (50-75%) within Union County and surrounding areas, focusing on the Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Program (D-SNP/FIDE). The role requires advanced clinical judgment, critical thinking, and effective communication skills to coordinate care and advocate for members' needs, ensuring they receive appropriate services and support for their overall wellness.

Responsibilities

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Develop, implement, and promote health service strategies and policies.
• Coordinate and collaborate care with members, authorized representatives, and care teams.
• Schedule and attend interdisciplinary meetings to advocate for members.
• Document accurately and timely in the member's electronic health record.
• Mentor new hires once proficiency is attained in the role.

Requirements

• 3+ years of clinical practice experience in a hospital or alternative care setting.
• Active and unrestricted RN license in NJ.
• Willingness and ability to travel 50-75% in Union County and surrounding counties.
• Successful completion of NJ Choice certification is a contingency of employment.
