The position involves coordinating the overall interdisciplinary plan of care for patients, from admission to discharge. The individual will monitor care and act as a liaison between the patient/family, healthcare personnel, and insurers. Responsibilities include evaluating the needs of the patient, assessing available resources, and recommending and facilitating plans for optimal outcomes. The role also entails developing discharge plans that provide the best available resources to meet ongoing patient needs and encourage compliance with medical advice. Additionally, the individual will identify patient care issues and suggest revisions to or new clinical pathways to improve the quality of care. There may also be responsibilities related to conducting utilization reviews.
