About the position

The Case Manager at The George Washington University Hospital is responsible for overseeing the comprehensive care of patients from admission through post-discharge. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating patient care plans to ensure effective transitions across the healthcare continuum. The Case Manager collaborates with interdisciplinary teams to manage complex patient needs, aiming for quality and cost-effective outcomes while participating in quality improvement initiatives.

Responsibilities

  • Assess and evaluate patients' needs upon admission and throughout their care journey.
  • Develop and implement individualized care plans in collaboration with healthcare teams.
  • Coordinate discharge planning to ensure a seamless transition of care.
  • Monitor patient progress and adjust care plans as necessary to meet evolving needs.
  • Facilitate communication among interdisciplinary staff and external partners.
  • Participate in quality improvement initiatives related to patient care management.

Requirements

  • Registered Nurse (RN) with a Bachelor of Science in Nursing (BSN) required.
  • Current licensure in the District of Columbia is required.
  • 2-3 years of recent acute care experience is necessary.
  • Case Management Certification is preferred.
  • Effective oral and written communication skills are essential.
  • Strong organizational and problem-solving skills are required.
  • Ability to maintain confidentiality and adapt to changing priorities.

Nice-to-haves

  • Experience in case management or care coordination is a plus.
  • Familiarity with healthcare regulations and compliance standards.
