Responsibilities
- Deliver the most remarkable patient experience in every dimension, every time.
- Collaborate with physicians and other team members in the discharge planning process.
- Work with the MSW for guidance on psychosocially complex cases.

Requirements
- Associates degree in Nursing or BSN required.
- Current RN licensure in state of employment required.
- Two or more years healthcare experience preferred.
- Case Management or related experience preferred.

- Bachelor's degree preferred.
- Weekend and holiday coverage.
