About the position
The Case Manager (RN or LPN) is responsible for coordinating skilled care for residents according to physician orders, ensuring effective communication with the interdisciplinary team, residents, and their families. This role involves conducting assessments, managing admissions, and overseeing ongoing care and discharge planning, while maintaining professional competence and participating in quality assurance processes.
Responsibilities
- Conduct on-site assessments and verify payer sources during pre-admission.
- Complete pre-admission screening forms and pharmacy/med surge quotes.
- Review and initiate orders upon admission, communicating with care community personnel.
- Coordinate residents' MDS's and Care Conferences.
- Participate in the QA process and the 'Nurse On-call' program.
- Maintain ongoing communication regarding resident goals, progress, and discharge planning.
- Oversee skilled residents' care and collaborate closely with the Office Manager.
Requirements
- Graduation from an accredited school of Nursing.
- Current license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with the State Board of Nursing.
- CPR certification recommended.
