About the position

The Case Manager at Select Specialty Hospital is responsible for overseeing utilization reviews, resource management, discharge planning, treatment plan management, and financial management for critically ill or post-ICU patients. This role involves collaborating with a multidisciplinary team to ensure safe and timely discharge plans while maintaining compliance with facility policies and maximizing reimbursement for services provided. The Case Manager plays a vital role in enhancing patient care and quality of life within the community.

Responsibilities

• Conduct utilization reviews and manage resources effectively.
• Develop and implement patient-specific discharge plans in a timely manner.
• Perform verification of utilization criteria reviews.
• Build relationships with payor sources to ensure proper reimbursement for services.
• Demonstrate compliance with Utilization Management policies and procedures.
• Coordinate UR compliance with Quality Management to meet licensure and accreditation requirements.
• Maintain fiscal responsibilities and maximize insurance benefit coverage for patients.
• Facilitate multi-disciplinary team meetings with healthcare professionals.

Requirements

• Current licensure in a clinical discipline (Nurse or Respiratory Therapist) or a certified Social Work license per state guidelines.
• Previous experience in critical care or acute care settings (ICU, step-down, med surg, vents).

Nice-to-haves

• Specific experience in Care Management and Discharge Planning.
• Working knowledge of the insurance industry and government reimbursement.
