About the position

The Case Manager (CM) position at Houston Methodist Willowbrook Hospital is a full-time role for a registered nurse (RN) responsible for comprehensive case management, including care transitions and discharge planning for a targeted patient population. The CM collaborates with physicians and the interprofessional healthcare team to ensure quality care and achieve desired treatment outcomes, while also managing the discharge planning process and ensuring medical necessity for admissions and continued stays.

Responsibilities

• Plan and manage case management for assigned patients, including care transitions and discharge planning.
• Collaborate with the healthcare team to facilitate quality care and achieve treatment outcomes.
• Communicate effectively with patients, families, and healthcare team members regarding patient care.
• Conduct thorough assessments of patients and participate in daily Care Coordination Rounds.
• Facilitate discharge planning activities and collaborate with social workers and other team members.
• Manage patient assignments and anticipate potential problems during care delivery.
• Document assessments and interventions accurately and efficiently.
• Review medical necessity for admissions and continued stays, ensuring compliance with guidelines.

Requirements

• Valid RN License in Texas or Compact State Licensure within 60 days.
• Bachelor's degree preferred.
• Three years of hospital nursing clinical experience.
• Experience in case management preferred.
• Knowledge of Medicare, Medicaid, and Managed Care requirements.
• Strong assessment, organizational, and problem-solving skills.
• Proficiency in Microsoft Office Suite.
