About the position

The Case Manager RN at ScionHealth is responsible for monitoring and managing patient care to ensure continuity, optimal outcomes, and patient satisfaction. This role involves collaborating with nursing staff and multidisciplinary teams to evaluate patient status, address care-related inquiries, and participate in discharge planning. The Case Manager RN plays a crucial role in enhancing patient experiences and ensuring compliance with healthcare standards.

Responsibilities

• Monitor and manage patient care to promote continuity and optimal patient outcomes.
• Consult with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of medical care.
• Respond to questions and complaints from patients, family members, and payors regarding care.
• Participate in discharge planning, including coordinating patient transfers and community resources.
• Provide discharge education and resource referrals to patients.
• Perform chart reviews to identify issues with service delivery, patient outcomes, and compliance.

Requirements

• WV Registered Nurse, WV LPN or WV Social Worker Licensure (one year to obtain a permanent license).
• Critical thinking skills and decisive judgment.
• Ability to work with minimal supervision in a stressful environment.
• Working knowledge of approved criteria for assessment and decision making.
• Ability to communicate effectively both orally and in writing.

Nice-to-haves

• Minimum four (4) years of hospital/clinical experience.
• BLS certification required within 30 days of hire.
