About the position

The Case Manager RN at Somatus is responsible for ensuring continuity of care for patients with chronic kidney disease in both inpatient and outpatient settings. This role involves utilizing nursing knowledge and skills to facilitate patient care, coordinate treatment plans, and ensure appropriate resource utilization to achieve quality patient outcomes. The position is fully remote, requiring effective communication with patients, families, and healthcare teams, while also managing care plans and documentation in electronic systems.

Responsibilities

  • Exhibit superior customer service skills in all interactions with patients and team members.
  • Prioritize patient care needs during initial visits and address emerging issues promptly.
  • Conduct virtual meetings with patients, families, and caregivers to discuss care and treatment plans.
  • Identify and assist with follow-up for high-risk patients in various care settings.
  • Consult with physicians and team members to implement care plans effectively.
  • Coordinate treatment plans with the care team and triage interventions appropriately.
  • Utilize established protocols to optimize clinical outcomes in disease management.
  • Monitor and coach patients using motivational interviewing techniques to enhance self-management.
  • Oversee discharge provisions, including follow-up appointments and home health services, to maintain continuity of care.
  • Maintain effective communication with healthcare providers, patients, and families.
  • Assist members in maximizing their health plan benefits.
  • Participate in Care Management Coordination Committee meetings actively.
  • Document patient information and care management plans in electronic health records.
  • Coordinate care with the interdisciplinary team for assigned patient caseloads.
  • Adhere to departmental policies and procedures.

Requirements

  • Active RN license in current state of residence with the ability to qualify for additional state licenses as requested.
  • 2+ years of nursing experience in a hospital, acute care, or direct care setting.
  • Experience in renal, chronic kidney disease, or dialysis care as a primary focus.
  • Proficiency in MS Office (Word, Excel, PowerPoint, Outlook) and ability to multitask in a Windows environment.
  • Access to a dedicated workspace at home for remote work setup.
  • Ability to work the specified schedule, including evening hours.
  • Residence in an area with high-speed internet access.

Nice-to-haves

  • BSN degree
  • Active New York RN license
  • Certified Case Manager (CCM) certification
  • Experience as a diabetic educator
  • ICU, cardiology, or critical care experience
  • Telephonic case management experience
  • Experience with discharge planning
  • Knowledge of hypertension and/or diabetes.
