About the position

The Case Manager, RN at Oklahoma City Indian Clinic is responsible for providing professional case management in a patient-centered medical home setting, focusing on the American Indian population. This role involves coordinating patient care, managing referrals, and ensuring effective communication among patients, families, and healthcare teams. The ideal candidate will be compassionate, energetic, and adaptable to a fast-paced environment, contributing to the clinic's mission of delivering excellent healthcare services.

Responsibilities

  • Report and notify patients of pap smear results and facilitate transfer of obstetric care.
  • Follow the Nursing and Case Management process including assessment, planning, implementation, coordination, and interaction through the referral process.
  • Coordinate and manage communication with patients, families, and the healthcare team to access healthcare services not available at OKCIC.
  • Ensure services support quality patient communication, education, financial, and tribal components.
  • Record in EHR and RPMS accurately and timely.
  • Process referrals accurately and in a timely manner.
  • Communicate with primary care providers regarding patient referrals to assess outcomes for further treatment.
  • Provide patient education regarding the purpose of treatment/referral and intended outcomes.
  • Document patient progress through the continuum of care.
  • Provide nursing support for physicians, nurse practitioners, and physician assistants.
  • Assess patient needs to coordinate and manage patient care within the clinic, including triage and scheduling appointments.
  • Obtain physical assessments and medical histories; communicate patient needs to primary care providers.
  • Manage team approach nursing and coordinate with other disciplines to provide quality care.
  • Perform specialized skills as per clinic policies and procedures.
  • Participate in assigned duties as designated by the VP of Nursing, including committee participation and equipment inspections.
  • Maintain professional knowledge and proficiency through continuing education, in-services, and workshops.

Requirements

  • Current Oklahoma Nursing License in good standing with the OK State Board of Nursing.
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing required.
  • Current CPR certification (or must be obtained within 6 months of hire).
  • Basic knowledge of managing women's health and pediatric patients preferred.
  • Knowledge of Resource and Patient Management System (RPMS) and Electronic Health Record (EHR).

Nice-to-haves

  • Experience in managing women's health and pediatric patients.
  • Familiarity with RPMS and EHR systems.
