About the position

The Ryan White Case Manager provides a wide range of psychosocial services to HIV+ patients referred by Legacy providers. This role focuses on case management, patient advocacy, and assistance with medication adherence, ensuring that patients receive the necessary support to improve their health outcomes. This is a grant-funded position aimed at driving healthy change in the community.

Responsibilities

• Identify and screen HIV+ individuals referred for social service needs.
• Assist clients in obtaining required documentation for program eligibility.
• Help patients complete drug assistance program eligibility applications.
• Enter data into the Harris County Ryan White Centralized Patient Care Data Management System (CPCDMS).
• Facilitate the transfer of CPCDMS record ownership for patients transferring care.
• Educate patients on services covered under Ryan White Part-A and coverage caps.
• Coordinate the collection of Ryan White CPCDMS eligibility supportive documentation.
• Assist with the preparation for the Annual Ryan White Part - A Audit.
• Assess each client's medical and psychosocial history and current service needs.

Requirements

• Master's degree in social work is required.
• Current Texas Licensed Master of Social Work (LMSW) is required.
• One year of case management experience with people living with HIV or related issues is required.
• Working knowledge of community resources and treatment issues related to HIV is necessary.
• Ability to manage multiple deadlines and prioritize tasks is essential.
• Demonstrated competency with an EHRS system.
