About the position

The Case Manager at the Center for Urban Community Services (CUCS) is responsible for assisting clients in obtaining housing and supporting them in leading fulfilling lives within the community. This role involves working closely with a designated group of clients, ensuring that the team meets its contractual obligations, including achieving housing placement goals, while embodying the core values and vision of CUCS.

Responsibilities

• Assist clients in obtaining housing.
• Support clients in living full and satisfying lives in the community.
• Ensure the team meets contractual obligations and housing placement goals.
• Work effectively as part of a multidisciplinary team.

Requirements

• High school diploma and 2 years of experience for CM 1.
• Bachelor's degree or high school diploma or equivalent and 4 years of relevant work experience for CM 2.
• BSW and 1 year relevant experience (excluding fieldwork) or Bachelor's degree and 2 years relevant experience or high school diploma and 6 years relevant experience for CM 3.
• 3 years as CM 3 (internal only) for CM 4.
• 3 years as CM 4 (internal only) for CM 5.
• Demonstrated ability to provide services to a specialized population.
• Computer literacy and good written and verbal communication skills.
