About the position
The Service Coordinator at Pettis County Board of Services for the Developmentally Disabled (dba) MPOWER is responsible for empowering individuals to manage their lives by coordinating services, supports, and funding. This full-time position involves developing Individual Support Plans (ISP), collaborating with various agencies, and ensuring that the needs of individuals are met through effective communication and organizational skills. The role emphasizes a supportive and team-oriented environment, with opportunities for flexible and hybrid work arrangements.
Responsibilities
- Pursues services, supports, and funding for individuals served to aid in progress towards identified goals.
- Collaborates to secure information contributing to the individual's current status for ISP development.
- Maintains knowledge of community resources and facilitates access for individuals served.
- Develops, monitors, and maintains records regarding services rendered and assesses their effectiveness in relation to ISP goals.
- Serves as a liaison between multiple agencies and team members to address individual needs.
- Organizes and facilitates team meetings to discuss progress and modifications to services in the ISP.
- Works collaboratively with others to ensure effective service delivery.
Requirements
- Must have a valid Driver's License and automobile insurance.
- Must have a Registered Nurse license and/or a Bachelor's degree from an accredited college or university.
- Strong customer service skills and effective communication skills are essential.
- Top-notch organizational skills and self-motivation are required.
