About the position

The Case Manager/Service Coordinator at United Disabilities Services Foundation (UDSF) plays a crucial role in supporting seniors, veterans, and individuals with disabilities to lead independent lives. This position involves identifying and monitoring services provided to participants, ensuring compliance with regulations, and adjusting services as necessary. The role requires collaboration with participants to help them achieve their goals in community settings, while also providing training and support to enhance service satisfaction.

Responsibilities

  • Identify and monitor services provided to participants through OLTL and applicable regulations.
  • Determine quality of services and verify adjustments needed in type, amount, scope, duration, and frequency of services.
  • Support individuals' goals to live independently in the community and establish natural connections.
  • Assist participants in receiving services within established time frames and standards.
  • Provide necessary training and support to participants and consumer employers regarding direct care services.
  • Prepare and complete all required forms and documentation in HCSIS, SC Database, and HALO system.
  • Complete Daily Activity Reports and Allocations to ensure appropriate program billing and allocation.

Requirements

  • Bachelor's Degree with at least 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or another behavioral science.
  • One year of case management experience, or a combination of experience and training totaling four years with relevant education.
